The 2010 Youth Bowling Tournament can be held any weekend from January through April 2010 in your Nest or District. Only one set of scores is acceptable for each participant.

Download The Entry Application (Microsoft Excel). Fill out the Entry Application and save it as your first name and last name.

Once you have filled out the application, click here to attacht it to an e-mail and send it to Chris Puskar.

All Instructors/Instructresses or anyone interested in entering the tournament should email me for an entry form at cpuskar@polishfalcons.org. I will then email you an entry form spreadsheet, which you will complete and return to me via email.

Each entry should also include the Instructor or Instructress information and the date and place where the bowling occurred as stated on the spreadsheet. I would also like you to try and send at least one picture of your group for the Sokol Polski.

ALL ENTRIES MUST BE EMAILED and CHECKS POSTMARKED BY
MAY 1, 2010.
